Waiver
I understand that participation in Wolf School LLC football training, drills, conditioning, and related activities involves inherent risks, including but not limited to falls, collisions, sprains, fractures, concussions, heat-related illness, permanent injury, or death.
I certify that the participant is physically able to participate in these activities and will notify Wolf School LLC of any medical conditions, injuries, allergies, or limitations that may affect participation.
In consideration of participating in Wolf School LLC programs, I voluntarily assume all risks associated with participation and release, waive, and hold harmless Wolf School LLC, its owners, coaches, employees, volunteers, and representatives from any claims, demands, or liability for injuries, damages, or losses arising from participation, except where prohibited by law or caused by gross negligence or willful misconduct.
I understand that football is a contact sport and that protective equipment does not eliminate the risk of injury.
I authorize Wolf School LLC to seek emergency medical treatment for the participant if I cannot be reached. I understand that I am responsible for any resulting medical expenses.
I understand that I am responsible for ensuring the participant arrives with appropriate athletic clothing, cleats (if applicable), and adequate hydration.
I have read this waiver, understand its contents, and voluntarily agree to its terms.
